Riders
(Available at an additional

Total Disability-Sickness Rider and Total
Disability-Sickness Rider – Non-Occupational
Pays a monthly benefit of up to $3,000* for total disability due to
sickness (including maternity).
Emergency Accident Rider
Pays actual expenses incurred for
emergency care of a covered injury up to the maximum rider benefit.
Issue amounts available up to $200.
Accidental Death, Dismemberment
and Paralysis Benefit Rider
Pays a death benefit if a covered injury results in death (within 90
days of the covered injury) or if a covered injury results in
irrevocable loss of sight or total loss of use or severance of hands
or feet. Maximum issue of up to $50,000 per employee, $25,000 for
spouse and $5,000 per child.
Injury Hospital Indemnity Rider
Provides a daily benefit for each day an insured person is
hospitalized due to a covered injury. Maximum issue of up to
$4,500 per month.
Hospital Indemnity Rider
Provides a daily benefit for each day an insured person is
hospitalized due to a covered injury or sickness. Maximum issue
of up to $4,500 per month.
NOTE: Only ONE (not both) of
the above two indented riders is available with this plan.
Outpatient Sickness Rider
Pays the actual expenses incurred up to the maximum benefit amount
for outpatient treatment received in a physician’s office or in an
ambulatory surgical center due to sickness. Up to two times the
maximum benefit amount if the outpatient treatment was received in
the outpatient department or emergency room of a hospital. Maximum
issue of up to $100 per occurrence.
Intensive Care Unit Rider
Provides a daily benefit if an insured person is confined to a
hospital’s intensive care unit, up to a maximum of 20 days per
period of confinement. Maximum issue of five times the base policy
(not to exceed $1,500 per day).
Private Duty Nurse Rider
Provides a daily benefit for each day an insured person requires the
services of a private duty nurse at least 8 hours a day
while confined in a hospital or to the primary
residence due to a covered injury or sickness, up to a maximum of 30
days per period of confinement. Maximum issue of $100 per day.
Surgical and Anesthesia Benefits Rider
Provides a benefit if an insured person requires surgery and
anesthesia due to a covered injury or sickness. Maximum issue of
$5,000.
NOTE: The Surgical and
Anesthesia Benefits Rider can NOT be purchased in
conjunction with the Surgical, Anesthesia and ADDITIONAL
Benefits Rider.
Removal of Integration of Benefits Rider
Eliminates coordination of benefits with Worker’s Compensation and
other Employer Liability Benefits.
Surgical, Anesthesia and Additional Benefits Rider
Provides a benefit if an insured person
requires surgery and anesthesia due to a covered injury or sickness,
mammography screening, pap smear, and PSA. Maximum issue of $5,000.
